We recognize that the healthcare industry, specifically outpatient physical therapy, has moved towards a quantity centered approach. This model leaves less personalized time with your provider and doing things that should be a home exercise program (HEP). This should not be the expectation – at Origin we’re here to change that.
We want our patients to feel comfortable knowing they are going to see the same healthcare team consistently that are informed about their individual case. With the Origin Physical Therapy model, it allows our team to provide quality care, meaningful information, and the tools and resources for long term functional improvement.

Anthony Tabor PT, DPT

Director of Operations
A native of Henderson, Anthony is passionate about rehabilitating the community in which he was raised, and in turn, is raising his family. Anthony attended the University of Nevada, Reno, for his Bachelor of Science in Public Health before receiving his Doctorate of Physical Therapy from Touro University of Nevada. Since 2012, Anthony has been practicing in the Las Vegas Valley rehabilitating various athletes including: UFC, NFL, NBA, MLB, PGA, NHL, Cirque and Spiegelworld. He has an educated understanding that patients with physical careers need specialized care and treatment plans to return to functional independence.

Susie Mireles

Administrative Director
Born and raised in El Paso, Texas, Susie has called Las Vegas home for almost 30 years. Her health administration career began as a receptionist at the renowned D. Keith Kleven Institute of Physical Therapy & Rehabilitation. At the Kleven Institute, Susie quickly showed a talent for streamlining office procedures and giving exemplary service to every single patient. Under the mentorship of Keith himself, Susie excelled and was subsequently promoted to Business Manager. Susie has continued to expand her administrative knowledge and immersed herself into credentialing, insurances, and overall operations. Susie's passion is to help others and give back to our community. When not at the office, she enjoys spending time with her wonderful family.

Taylor Tabor

Marketing Director
A native of Myrtle Beach, SC, Taylor grew up as a competitive dancer studying multiple dance disciplines including ballet, classical jazz, and ballroom. Her professional career kicked off full time and she performed with several family theaters around Myrtle Beach, SC and Lancaster, PA. This segued to performing on the high seas with Holland America Line and Seabourn. After visiting close to 70 countries, Taylor made her home base right here in Las Vegas, joining the cast of Limelight and Vegas! The Show. Following a decade of performing full time, Taylor's final curtain call arrived, leading her to a new career in medical marketing. Taylor channels her real life experience with various injuries and interventions and has been educating our community on the long term benefits of physical therapy for the past 8 years. Taylor enjoys spending her free time cooking and relaxing with her family; and is usually on the go with a very active toddler.

Ryan Sanchez PT, DPT

Physical Therapist
Ryan graduated from California State University, Long Beach with a B.S. in Kinesiology and concentration in Sport Psychology. He went on to receive his Doctorate of Physical Therapy from Touro University Nevada right here in Henderson. His passion for exercise and movement stemmed from growing up playing competitive club soccer. Ryan has been a certified personal trainer since 2017, with a background in 1-on-1 and small group training. He enjoys helping people achieve all aspects of their health goals whether the goal is to relearn to walk, increase shoulder strength, learn how to squat pain-free, or improve athletic performance. In his free time he enjoys doing CrossFit, playing golf, hiking/running with his dog, and snowboarding.

Harper Luecke Keese PT, DPT

Physical Therapist
Harper graduated with a B.S. in Kinesiology with a concentration in Motor Behavior (psychology minor) from Texas A&M University. She then attended Texas Woman's University in Dallas, TX where she earned her Doctorate of Physical Therapy. As a lifelong dancer with extensive competitive and collegiate training, she provides a tailored perspective on orthopedic rehabilitation. Harper specializes in sports, dance, balance and vestibular, and degenerative neurological conditions. Harper's goal is to use real life experience, clinical training, and the love of continual learning to provide unmatched and personalized care. She has a passion for helping patients facilitate a healthy and fulfilling life. In her personal time, Harper enjoys relaxing at the lake and trying new restaurants with friends and family. Organizations & Associations: International Association of Dance Medicine and Science, Nevada Physical Therapy Association (NPTA), American Physical Therapy (APTA).

Jeremy Tu PT, DPT

Physical Therapist
Jeremy attended UCLA where he received his Bachelor of Science in Psychobiology before graduating from Touro University Nevada in Henderson, NV with his Doctorate of Physical Therapy. During his time at Touro he honed his craft in a variety of settings, from outpatient orthopedics to inpatient hospitals and skilled nursing facilities. In all of these settings he integrated patient care with exercise principles he learned over the years as a dedicated weightlifter. He believes "movement is medicine" and encourages all of his patients to do so in their own unique way. When Jeremy is off the clock, you'll find him on a bench press, Zion National Park, or at a Golden Knights game.

Garik Smith PTA

Physical Therapy Assistant
Originally from Rhode Island, Garik has been a Physical Therapy Assistant in Las Vegas since 2018. After attending the University of Rhode Island and New England Institute of Technology, Garik's outpatient physical therapy focus has been sports rehabilitation. He has had the privilege of working with professional (MLB, NFL, UFC, World Strongest Man, and more), collegiate, and high school athletes. Garik grew up playing soccer, baseball, and basketball which eventually contributed to his journey with physical therapy. He has furthered his education in the realm of sports taking courses on the psychological recovery of sports related injuries and is certified in Blood Flow Restriction Therapy. He also is experienced in shoulder rehabilitation, balance training, post operative rehabilitation, and continues to progress his skill set with manual therapy techniques. Growing up on a small island in New England, Garik has always had a love for the ocean which inspired his new puppy's name, Reef.
